Historia

Inkurajtimet e para të mjekësisë emergjente para spitalore datojnë në Luftën e Dytë Botërore kur mjekët privatë me karoca u përdorën si një mjet transporti.

Stacioni i ndihmës së parë u krijua për herë të parë pas luftës brenda poliklinikës së atëhershme të qytetit me dy mjekë në ndërrime.

Si një stacion i posaçëm si Ndihmëe shpejtë medicinal  u krijua më 1 korrik 1956 me katër ekipe. Më 1962 u futën stacionet e radios në automjetet sanitare.
Në vitin 1970, IPSH  u formua  Qendra Shëndetësore Shkup-Shkup dhe Ndihma e shpejtë medicinale është e integruar në të si një njësi punuese me një lokacion në Poliklinik Bukuresht ku është organizuar sot.

Në vitin 2001, Shërbimi u zgjerua për të krijuar një qendër dispeçerie, një Punkt-Jane Sandanski brenda poliklinikës Jane Sandanski dhe një Punkt  – Çair  në kuadër  të Poliklinikës Çair .

Qëllimi i krijimit të pikave është:
• përmirësimin e shpejtësisë së ndërhyrjes, që nënkupton shkurtimin maksimal të kohës së nevojshme për të arritur dhe për të ofruar ndihmë për të sëmurët ose të lënduarit nga momenti i pranimit të thirrjes; 
• Ulje të ndjeshme në koston e punës së përditshme, kryesisht duke reduktuar kilometrazhin bosh dhe duke reduktuar konsumin e karburantit;
• formimi i një tërësie në mbulimin e qytetit me ekipe në terren.
Në vitin 2007, pika e katërt-Çento u krijua brenda Poliklinikës Çento, duke bërë një tërësi në mbulimin e qytetit sipas shpërndarjes së qytetit dhe mbulimit të autostradës.

Kufijtë në mes të pikave nuk janë përcaktuar në mënyrë strikte dhe thirrjet merren sipas shkallës së urgjencës së ekipeve nga pika më e afërt tjetër nëse ekipet nga ajo pikë janë në një intervistë.

Nga viti 2006 deri në vitin 2008 është bërë zëvendësimi i plotë i automjeteve të vjetra sanitare me ato të reja, të punësuarit marrin uniforma me një përcaktim të posaçëm të Shërbimit, formohet një departament trajnimi dhe po ndërtohet një hapësirë e re parkimi për automjetet sanitare.

Sot Shërbimi është i organizuar me:

Shërbimet shëndetësore të ofruara nga Shërbimi janë:
– ndërhyrjet e shkallës së parë dhe të dytë të urgjencës
– transport në qytet, shtet dhe jashtë tij
– garanci mjekësore të manifestimeve me karakter të ndryshëm

1.QENDRA PËR TRIJAZH DISPEÇERIKE  -baza Kaprosh

Qendra e dispeçerike është një pjesë koordinuese në të cilën të gjitha thirrjet pranohen në nivelin e qytetit të Shkupit, shpërndahen në ekipe në varësi të asaj se nga  cila pjesë e qytetit është thirrja dhe nga cili Punkt  do të ndërhyjë ekipi.

Në këtë mënyrë, ekziston një koordinim unik dhe shikueshmëri e plotë e ekipeve, dhe nëse ekipet nga një pikë tashmë janë në ndërhyrje, do të dërgohet ekuipazhi më i afërt nga pika fqinje.

Qendra e dispeçerike  përbëhet nga:
• Pjesa e pranimit të thirrjeve
•  Pjesa për shpërndarjen e thirrjes së ekipeve në terren

Në pjesën për thirrjet pranuese ka katër linja telefonike nëpërmjet të cilave të marrë të gjitha thirrjet në qytetin e Shkupit, registrifon  të cilat regjistrojnë bisedat ,dhe dy mjekë të cilët  në një bisedë të shkurtër me thirrësit, d përdorin  pyetjet  pa përgjigje të sugjeruara dhe sipas të dhënave të marra nga natyra e sëmundjes ose të vjellat , të marrë një vendim për veprime të mëtejshme.

 

Mundësitë janë si më poshtë:
• Thirrja pranohet si nevojë për ndërhyrje emergjente mjekësore, ndërhyrje emergjente e shkallës së dytë dhe  përshkruhet diagnozë  ose një përshkrim i shkurtër të simptomave ose dëmtimeve sipas të cilave përcaktohet shkalla e urgjencës,  shënëhen gjeneralet , treguesi i saktë i vendit të ngjarjes  dhe koha e pranimit të thirrjes.
•  Nëse thirrja nuk është për ndihmë të shpejtë , thirrësi drejtohet te mjeku amë , vizitë shtëpiake ose shërbimi kujdestarik i natës.
• Jepen këshilla të përshtatshme

Frekuenca dhe shumëllojshmëria e thirrjeve është e tillë që kërkon shumë përqendrim, përvojë dhe aftësi për komunikim verbal dhe njohuri të simptomave që tregojnë urgjencën. Për shkak të përgjegjësisë së madhe, këtë pozitë mund të punojnë vetëm ata mjek të cilët kanë punuar në teren së paku tre vjet.

Përveç këtyre pyetjeve mund të parashtrohen edhe pyetje plotësuese.
Pyetjet bëhen në mënyrë që të transferohen tek ekipi se çfarë lloj ndërhyrje duhet të ndërhyjë dhe nëse duhet të ketë pajisje mjekësore të veçantë përveç pajisjes standarde mjekësore.

Nëse thirrja është pranuar duhet të jepet adresa e saktë , numri I telefonit se si mund të realizohet kontakti në rast vështirësie për gjetjen e adresës . Me ardhjen e ekipit, këshillat e mjekut trajtohen në telefon dhe broshura ,si dhe përgatitet dokumentacioni shëndetësorë I të sëmurit.

Të gjitha thirrjet e marra janë dorëzuar në pjesën për shpërndarjen e thirrjeve ku dy dispeçer punojnë dhe ato i dorëzohen ekipeve sipas vendit të ngjarjes  dhe cili ekip është më i afërti.

Shkëputja e thirrjeve përfshin punën me një numër të madh të të dhënave ose me elementët e thirrjes individuale dhe përdorimin e ekipeve në dispozicion në mënyrën optimale. Dispeçerët janë punëtorë mjekësorë që kanë përvojë sumëvjeçare  në terren, e njohin qytetin dhe kanë aftësinë për të përdorur me shpejtësi të dhënat e dhëna në favor të vendosjes adekuate të ekipeve.
Procesi i marrjes dhe transmetimit të thirrjes mund të zgjasë me sekonda në minuta në varësi të aplikantit.
 

Ekipe Medicinale
Ekipet mjekësore në terren përbëhen nga një mjek, një infermiere / teknik dhe një shofer.

Gjatë 24 orëve, 365 ditë ndihme mjekësore emergjente ofrohen nga tetë-nëntë  ekipe në territorin e qytetit të Shkupit. Në përgjithësi, dy nga ekipet janë të vendosura në pikat Jane Sandanski dhe Çair, një pikë ekuipazhi Çento dhe katër ekipe në bazën e shërbimit Karpos, megjithatë orari i ekipit varet nga nevoja e qytetit dhe mund të ndryshohet gjatë ndrrimit.

Në rast nevoje, numri i ekuipazheve rritet.Çdo automjet sanitar i Shërbimit të NSHM ( ndihma e shpejtë medicinale ) është i pajisur me pajisje mjekësore standarde për kujdesin emergjent mjekësor. Shërbimi Mjekësor i Emergjencave kryen transport mjekësor për pacientët në qytet, në vend dhe jashtë vendit, sipas një treguesi mjekësor ose sipas kërkesës së pacientit.

Për transfertat në qytet është e regjistruar në numrin 194, dhe në vend ose jashtë saj me infermieren kryesore.
 

Sigurimi medicinal
Shërbimi siguron ofrimin mjekësore të tubimeve dhe manifestimeve me karakter të ndryshëm (sport, kulturor, politik). Siguria kryhet me kërkesë të organizatorit të manifestimeve ose mbledhjeve dhe duhet të raportohet së paku dy ditë para ngjarjes.
